What is the 2022 FedEx Claim Form?
FedEx’s claim form can be used for FedEx Express, FedEx Ground, and FedEx Freight shipments within the U.S. from the U.S. to other countries, or for shipments from Canada to the U.S. in 2022. Claim forms are available for shipments not received or when they are damaged or missing content.

What types of claims does FedEx accept?
Before filling out a claim form, it’s helpful to know exactly what types of claims FedEx accepts so you don’t waste time.
FedEx’s Online Claims supports FedEx Express, FedEx Ground, and FedEx Freight claims for the following shipments:
- Shipping within the U.S.
- U.S. Shipping to Other Countries
- Canada to USA
To file a claim for a shipment from outside the United States, please contact FedEx International Customer Service at 1.800.463.3339.
In terms of the types of claims accepted, FedEx’s online platform offers the following types of claim options:
- Shipment not received
- Shipping Missing Content
- Shipment damaged
- Cash on delivery not received
What do I need to fill out the FedEx claim form?
You will need your tracking number or PRO number to fill out the FedEx claim form.
Additionally, all claims filed through FedEx must be accompanied by documentation supporting the amount claimed.
Such documents may include the following:
- Original manufacturer or purchase invoice
- Repair estimate or invoice
- expense report
- Evaluate
Finally, it would be helpful to provide photos to show evidence of damage and/or missing content when appropriate.

How do I fill out the FedEx Claim Form?
To complete a FedEx claim form for a damaged, missing or missing shipment, you can submit it online (most commonly), in writing, or by phone.

Please note that no documentation is required for claims with a declared value of less than $100.

Step 3: Submit your online claim form
Review the information you provided, submit a claim form, and print a case number page for your records.
Step 4: Schedule an inspection or do it yourself
FedEx will contact you if your claim needs inspection. You can schedule an inspection with a FedEx representative, or you can schedule an inspection yourself.
Customers can self-check FedEx Express and FedEx Ground shipments with a declared value greater than $100 but less than $1,000.
To complete the self-inspection, download and complete the inspection report, attach a digital photo of the package, and submit the completed report online.
Step 5: Track Your Claim Status
Log in to your FedEx account to check the status of your claim.
To file a claim by mail:
Download the claim form and fill out the following information:

You will be in touch with a customer service representative who will fill out a claim form for you. After that, you will receive your claim case number.
If you need to submit supporting documents, you can do so online or by mail.
How long do you need to fill out the FedEx claim form?
If your claim relates to a lost, damaged, delayed or COD payment, you should file your claim within 21 calendar days of the delivery date.
You should file a claim for non-delivery or misdelivery of your shipment within 9 months of the date of shipment.
What happens when you submit a complaint form to FedEx?
You should hear back from FedEx within 7 to 10 business days of submitting your claim form.
When contacting you, FedEx may tell you that they need additional documentation to process your claim.
