On the left side of the form, you’ll see a box that says “I authorize FedEx to leave the package referenced below:”
Below, you should fill in the following information:
- tracking number
- The recipient’s name
- receiver’s address
- phone number
To the right of the form, you’ll see another box that says “at the location specified below.” Here, you should tick the box indicating where you want the package to be left.
Options include front door, back door, other or immediate neighbor/building manager.
If you choose the immediate neighbor or building manager option, you must provide their address.
Additionally, that person must be present to sign for the package when it arrives.
At the bottom of the form, you must print your name, signature and date.
Do not complete the section at the bottom of the form labeled “FedEx Use Only”.
